Pylorus-preserving gastrectomy for early gastric cancer

Abstract
Physical results after pylorus preserving gastrectomy(PPG) in early gastric cancer patients were evluated.

Methods : From August 1996 to March 1998, sixteen patients underwent PPG, and 17 patients underwent conventional distal gastrectomy with Billroth I anastomosis(DG), Patients undergoing the PPG and DG procedure were assessed 1 year after their surgical procedure, changes in body weight, food intake volume and abdominal symptoms according gy1estionaire, gastric emtying test by acetaminophen method and gastroscopic finding were compared beween the two groups.

Results : There were no significant differences in body weight ratio, amount of food taken in a meal between groups. Patients who had. PPG had fewer postoperative abdominal symptoms than those who underwent DG. After DG, emptying was much more rapid. Gastroscopy revealed that mucosa of the stomach remnant after PPG was less abnormal than after DG, But food stasis was more frequent after PPG.

Conclusions : PPG is a more physiological operation than conventional DG and should be used in carefully selected patient with early gastric cancer to improve their quality of life.
